2025 MaTOS Lung | Session VI | Locally Advanced NSCLC-Unresectable Case-Based Panel Discussion

Overview

Dr. Fidler presented Case 1 where the panel reviewed a young woman with unresectable, EGFR-mutant NSCLC, discussing neoadjuvant chemo with or without osimertinib, fertility goals, surgery feasibility, pregnancy timing, and MRD monitoring.
Case 2, presented by Dr. Massarelli, discussion covered a 74-year-old with COPD and T3N2M0 squamous NSCLC, not surgical. The panel debated non-surgical chemo-radiation, induction options, and maintenance therapy despite low PD-L1.
Dr. Velazquez-Manana presented Case 3, a 42-year-old with comorbidities and RLL lung cancer, high TMB, PD-L1 negative. After poor chemo response, the team debated options and moved to chemoradiation.
In Case 4, presented by Dr. Subramaniam, the panel reviewed advanced localized NSCLC with high PD-L1. Surgery was precluded by a pacemaker, so chemo-radiation plus durvalumab was used. Response was strong but complicated by pneumonitis, cardiac issues, and later recurrence.
